This tool comes from Rev. Glen Stevens, pastor of Crossroads Evangelical Free Church in Albert Lea, Minnesota. Thanks to him for sharing this.

This form is intentionally only 1 page, 2-sided. One side is to be filled out before the ministry/event, the other side after.
